What to expect on your first visit

During your initial evaluation, you will meet one on one with your physical therapist to create an individualized treatment plan based on symptoms, history, and the results of your range of motion and strength assessments.  You may also begin manual therapy, your exercise program, and receive therapeutic modalities.  Your visit should last 45 minutes to 1 hour.

What you should bring

Please wear comfortable clothing and bring your insurance card, doctor's referral (if you have one), and your completed paperwork.  All intake paperwork is provided below.  If you have any further questions on what to expect, please do not hesitate to contact our office!
